Transition Coordinator


We at Trusting Hearts LLC are here for you in many ways. As a transition coordinator we assist families who have a loved one or an individual who has been hospitalized or in a Nursing Home setting for more than 60 days and meets eligibility requirements return home to their families and friends. We assess and develop a plan with our clients to ensure we obtain the appropriate services and supports needed for a safe transition and to help prevent Re-Hospitalization .
